Introduction
Between 2007 and 2024, data reported by Members of the International Association of Oil & Gas Producers (IOGP) show that 175 fatalities occurred across 86 Process Safety Events (PSEs). In response, the IOGP developed the Process Safety Fundamentals (PSFs) to help companies reduce -and ultimately eliminate- fatal and high-severity process safety incidents.
Designed for those working in front-line operations, maintenance, and well activities, the PSFs are grounded in industry data and highlight the situations most likely to result in process safety fatalities. By promoting awareness and reinforcing safe behaviours, the PSFs aim to strengthen operational discipline and prevent serious incidents across the industry.
